FOB Access Application
This sanctuary is a safe space for all and the safety of our children and congregants is our collective responsibility.
It is our top priority at The Rhoda. (FOB applicants must be at least 18 years old)
APPLICATION PROCESS
1. Download the FOB Access Application form HERE
2. Fill out the form, sign it, and upload it below (upload button)
3. Download the volunteer form HERE to apply for a Criminal Record and Judicial Matters Check
- CRC applications can be completed online (10 minutes) via this LINK
- Please note that you will need to download and submit the volunteer letter we have provided you with to waive the application fee.
4. Submit your signed contract online via the form below and complete your application by bringing the following documents in person at the Rhoda Library (2871 St. Joseph BLVD.) during the admin office hours.
- A Criminal record and judicial matters check (electronic or print)
- A valid proof of ID (Driver's license or any government-issued ID with address)
- A 50$ security deposit fee
